Self -directed support (SDS) is the way people access care and support from their local Health and Social Care Partnership (HSCP) to live as independently as possible within their community.

It is a personalised way of accessing care and support to give people as much choice, control and flexibility as possible.

The modules aim to provide an overview of the SDS options and process as well as more specific advice and practical guidance and will benefit anyone interested in learning more.

Each module explains a different part of SDS in bitesized chunks. You can complete a whole module or jump to a specific section if you are looking for something in particular.

Introduction to Self-directed Support

This module is a short introduction to Self-Directed Support, with answers to the following key questions: 

  • What is SDS?

This module explains what options are available within an Option 1 budget and how to plan your support.  

It is for people who have:

  • Been assessed as eligible for Self-Directed Support 
  • Have had their budget agreed

Accessing Support without SDS

A collection of support services for individuals who meet the following criteria:

  • Have completed the SDS assessment but don’t qualify for support
  • Seek support from a service other than the Health and Social Care Partnership
  • Feel that the current SDS support does not meet all needs
  • Prefer alternative support while waiting for the SDS Package to be finalised and actioned
